We have recently reviewed ours and kept the 5×5 matrix, however the most important thing we did is add a set of descriptors for both Likelihood and Consequence to both remove subjectivity between people (your possible is my likely), and to be able to compare risks across the entire company (H&S vs Public Safety vs Network etc).
